A model application form is included in pdf format with a link opposite..  The form is necessarily comprehensive and employers are free to use modify it to suit their needs.

Use the information on the application form to decide who to interview.  Be careful that the interview criteria are fair and clear and apply them consistently.

 

The alternative to an application form is to request a CV.  CVs have the advantage that individuals are more likely to apply for a job, filling an application form out can be very off putting.  However it is much easier to compare applicants if a standard form has been used and application forms can gather the correct information.

 

Our advice is if you only expect a few applicants or the job is very specialised ask for CVs.  If a larger number of applicants are expected or the job is a standard one use an application form.
