This form should be used to keep a brief record of the
performance interview. Both
employee and manager should be given a copy of the form once it has been
completed. No further copies of the
form need be distributed unless the employee is unhappy with outcome of the
interview.
Section 1
YOUR JOB (This section to be completed by the employee before
the interview) List below the five main tasks in your job and the rough percentage of
time you spend on them:
Briefly describe below the other main tasks. After them again indicate the rough percentage of time you
spend on them.
Section 2.
YOUR PERFORMANCE (This section to be completed by the employee before
the interview) List below the your current performance objectives
(i.e. those set at the last interview and subsequently modified since).
Finally summarise how you think you did in delivering those objectives.
Sections 3-6 to
be completed by the Manager after the interview. Section 3.
REVIEW OF THE JOB STRUCTURE Are you happy with the structure of the job as set
out above?
If not indicate below the tasks that need to change:

REVIEW OF PERFORMANCE Briefly state below how well current performance
objectives were met:

PERFORMANCE OBJECTIVES Briefly state below the performance targets you would
like the employee to concentrate upon during the next review period:
Section 6.
PERFORMANCE IMPROVEMENT What help will the employee be provided with to meet
those performance targets (i.e. coaching, supervision, training etc)
